29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  On or about January 23, 2019, employees were working on wooden scaffolding and the platform/walkway was not at least 18 inches wide. This condition exposed the employees to a fall hazard of more than 10 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about January 23, 2019, employees were performing framing work from wooden scaffolding that was not provided with guardrails or other forms of fall protection. This condition exposed the employees to a fall hazard of more than 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about January 23, 2019, employees used a portable ladder to access scaffolding and the ladder side rails did not extend at least 3 feet above the landing surface. This condition exposed the employees to a fall hazard.
