29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Facility, 2204 Todd Road, Aliquippa, PA:  On or about January 23, 2019, employees perform maintenance and repair work on vehicles that are energized/powered without a written lock out/tag out program, training and periodic inspection.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Facility, 2204 Todd Road, Aliquippa, PA:  On or about January 23, 2019, two employees work in a maintenance shop with different chemicals such as, but not limited to; Datey All purpose cement, Heavy duty glass cleaner and flex seal and power tools such as, but not limited to, DeWalt circular saw, without a specific hazard assessment performed to determine what hazards the employees could encounter and what PPE would be required when working with them.
