29 CFR 1910.29(b)(2): Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members were not installed between the walking-working surface and the top edge of the guardrail system:      A.)  Foam Fabricators, Inc.; pre-expand area - On or about 1/24/19, and at times prior thereto, the company did not have a mid rail installed on a stairway.  The stairway had approximately 10 risers and led to an overhead storage area above the shipping/receiving office.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    A.)  Foam Fabricators, Inc.; maintenance department - On or about 1/24/19, and at times prior thereto, the company did not have a tongue guard installed on a Baldor bench grinder.  The grinder was used to trim metal and remove burrs from metal.
