29 CFR 1910.303(b)(1)(viii): Safety of equipment was not determined using considerations for other factors which contributed to the practical safeguarding of employees using or likely to come in contact with the  equipment:  a) 111 Valley Rd., Hamburg, PA - The #3 blow molding unit was not free from recognized electrical hazards. On or about January 24, 2019, employees performing production and maintenance activities are exposed to electrical shock and fire hazards from contact with machine parts that were not properly safeguarded for the following reasons:         1- open wire-nut terminations within machine.         b) 111 Valley Rd., Hamburg, PA - The #8 blow molding unit was not free from recognized electrical hazards. On or about January 24, 2019, employees performing production and maintenance activities are exposed to electrical shock and fire hazards from contact with machine parts that were not properly safeguarded for the following reasons:         1- broken conduit connections on west side of machine         2- flexible wiring not provided with outer protective jacket on west side of machine.         3- cracked timer cover exposing inside of control panel.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:  a) 111 Valley Rd., Hamburg, PA - On or about January 24, 2019, the #8 machine contained splices that were not covered with an insulation equivalent to that of the conductors or with an insulating device:       1- open splice electrical connections on east side of machine.          b) 111 Valley Rd., Hamburg, PA - 111 Valley Rd., Hamburg, PA - On or about January 24, 2019, the #9 machine contained splices that were not covered with an insulation equivalent to that of the conductors or with an insulating device:      1- non-bushed flexible wiring exiting control panel with open splices.     2- open splice connection on top of machine.     3- open splice connection on lower side of machine east side.        Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
