Castelli Development Corporation, LLC dba Castelli Oil & Gas Services, 983 Woodland Bigler Highway, Woodland, PA  16881, wash bay area, on or about January 25, 2019 and times prior thereto:  Employees are exposed to corrosive materials including, but not limited to, Polytech.  The plumbed emergency eye wash was not tested on a weekly basis.

Castelli Development Corporation, LLC dba Castelli Oil & Gas Services, 983 Woodland Bigler Highway, Woodland, PA  16881, on or about January 25, 2019 and times prior thereto:  Employees are provided with, and permitted to use, portable dry chemical fire extinguishers.  Employees did not have initial fire extinguisher training upon hire.
