29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a) New residential construction located at 3700 Villa Springs Circle, Second floor over garage: On or about 01/23/2019, and at times prior, the employer exposed employees to struck-by hazards while operating Hitachi Framing Nail Guns, in that protective eyewear was not provided to employees operating these tools at 120 PSI while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located at 3700 Villa Springs Circle, Upper Roof: On or about 01/23/2019, and at times prior, the employer exposed employees to fall hazards of approximately 8-feet, 1-inch to the lower level, in that a fall protection system was not provided to an employee installing plywood sheathing on a 12/12 pitched roof.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) New residential construction located at 3700 Villa Springs Circle, Temporary Electrical Service: On or about 01/23/2019, and at times prior, the employer failed to inspect all of the electrical cords prior to the start of the work shift. A 100 ft. 12-gauge extension cord used to power equipment on site was missing a ground pin and was not repaired or removed from service.
