29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Worksite - Employees were installing exterior panels on a three story commercial building while working from the bucket of a Sky Power 860 SJ Articulating Boom. The employees were approximately 15ft above the ground and were not tied off to the boom or basket; on or about 1/28/19. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Worksite - Employee was installing exterior paneling on the exterior of a three story commercial building, while crawling on the edge of a widow overhang. The employee was exposed to falling approximately 30ft to the ground below; on or about 1/28/19. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
