29 CFR 1910.178(l)(3)(i)(M): Powered industrial truck operators did not receive initial training on truck-related topics including operating instructions, warnings, or precautions listed in the operators manual for the types of vehicles that the employees were trained to operate.    On or about January 28, 2019, employees that used the Clark forklift (model GCX17E; s/n GX177E-0148-9345K0F) to load/unload semi-trailers were exposed to drop-off hazards in that they were not trained in operating instructions of securing the trailers with a dock lock system or wheel blocks.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. On or about January 24, 2019, the employer did not ensure that the Yang Iron Works lathe (mfg. no. 059684) located in the tool room was guarded to protect employees from rotating part hazards.     b. On or about January 27, 2019, the National shear, located in the tool room, was not guarded to protect employees from amputation injury.  The employer did not ensure the employees were adequately protected from pinch point hazards created by the blade and hold-down mechanism.
