29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  a) 970 Milwaukee Ave.- On or about January 26, 2019, the employer failed to provide a guard over the rotating shaft that projected more than one half the diameter of the shaft protruding from the salt spreader on Truck #434.  The shaft extended three inches and was not smooth.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
