29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  On or about 12/16/2018, at an existing commercial building located at 545 Production Avenue, Naples Florida, an employee was exposed to a fall hazard of approximately 4 feet, 8 inches to ground level, while performing stucco and lathing work from a scaffold that had only one row of 12-inch wide planks.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  On or about 12/16/2019 at an existing commercial building located at 545 Production Boulevard, Naples, FL, an employee was exposed to a fall hazard of approximately 4 feet, 8 inches to the ground level, while performing stucco and lathing work from a scaffold that was not erected under the supervision of a competent person.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about 12/16/2018, at an existing commercial building located at 545 Production Avenue, Naples Florida, the employer failed to provide training on the hazards of scaffolds to an employee who performed stucco and lathing work from a scaffold that had only a 12-inch wide platform.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about 12/16/2018, at an existing commercial building located at 545 Production Boulevard Naples, FL, the employer failed to provide training on the hazards related to ladders to an employee who placed a 6-foot A-frame stepladder on a 12-inch wide scaffold plank.
