29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a)  On or about January 23, 2019, and at times prior, employees were exposed to eye injury while working with and around pneumatic nail guns without eye protection.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) On January 23 2019, the exposing employer, Lopez Roofing, did not ensure employees, who were required to engage in re-roofing operations on a residential apartment complex, were protected from exposure to fall hazards of 32 feet and greater.   Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
