29 CFR 1910.37(e): The employer did not assure that employee alarm systems were  installed, maintained, and tested in accordance with the requirements of 29 CFR 1910.165(d):  (a) Garage Bay - On or about January 31, 2019, a First Alert carbon monoxide alarm Model No. CO606 was unplugged.  Potential sources of carbon monoxide included vehicles that were started and driving into and out of the garage bay.  (b) Living Room - On or about January 31, 2019, the employer failed to install a carbon monoxide alarm in the living room area which contained sleeping quarters.   Potential sources of carbon monoxide included vehicles that were started in the garage bay adjacent to the living room. (c) Living Room - On or about January 31, 2019, the employer failed to install a smoke alarm or equivalent in the living room area which contained sleeping quarters.

Ambulance A136 - On or about January 31, 2019, a fire extinguisher was overdue for the annual maintenance check. It was due in March 2018. b. Ambulance A137 - On or about January 31, 2019, a fire extinguisher was overdue for the annual maintenance check. It was due in February 2017.
