(b) Living Room - On or about January 31, 2019, the employer failed to install a carbon monoxide alarm in the living room area which contained sleeping quarters.   Potential sources of carbon monoxide included vehicles that were started in the garage bay adjacent to the living room. (c) Living Room - On or about January 31, 2019, the employer failed to install a smoke alarm or equivalent in the living room area which contained sleeping quarters.
