29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:    (a) TA Carpentry at 3890 Bridle Ridge Severance, CO 80549: On or before 1/31/19 the correcting, creating and exposing employer did not protect employee from injury in that safety glasses were not worn when operating pneumatic nail guns. An employee was sheathing the roof of the front porch. This condition exposed employees to eye injuries.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:     (a) TA Carpentry at 3890 Bridle Ridge Severance, CO 80549: On or before 1/31/19 the correcting, creating and exposing employer did not protect employees from injury in that electrical cords were not inspected prior to use and taken out of service if they were damaged. Two extension cords had multiple spots where bare wire was exposed due to damage. This condition exposed employees to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) TA Carpentry at 3890 Bridle Ridge Severance, CO 80549: On or before 1/31/19 the correcting, creating and exposing employer did not protect employees from injury in that an employee sheathing the roof of the front porch of a new single family home was not wearing fall protection. This exposed the employee to a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     (a) TA Carpentry at 3890 Bridle Ridge Severance, CO 80549: On or before 1/31/19 the correcting, creating and exposing employer did not protect employees from injury in that a portable extension ladder used to access the front porch roof did not extend at least three feet past the eave of the roof. This exposed the employee to a fall hazard of approximately 10 feet.
