29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employees amputation or an employees loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:   (a)  On or about January 31, 2019, at the work place located at 130 N. Hillcrest Drive, Sulphur Springs, TX, following the work-related hospitalization of an employee that occurred on January 7, 2019, the employer did not make an oral report to OSHA within 24 hours after the occurrence.
