29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) On or about January 30, 2019, on the east end and south end of the construction site located on 2740 N Westport Dr., Port Allen, LA 70767, the employer did not ensure that the scaffold had portable ladders attached.

29 CFR 1926.501(b)(2)(ii): Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail system, safety net system, or personal fall arrest system.   a) On or about January 30, 2019, on the east end and south end of the construction site located on 2740 N Westport Dr., Port Allen, LA 70767, the employer did not ensure that the employees had appropriate access to the roof. The employees traversed the roof without fall protection.
