Safety of equipment shall be determined using the following considerations:  29 CFR 1910.303(b)(1)(vi): Arcing effects   a. Located in the first and second floor kitchen areas, employees were exposed to an electrical shock hazard while plugging in the hot boxes into two 120 volt electrical receptacles that were arcing.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.   a. Located throughout the first and second floor kitchen areas, employees were exposed to an electrical shock hazard due to several 120 volt receptacles in use without a faceplate.      b. Located in the first floor kitchen area, the junction box under the garbage disposal sink had a cover that was not secured to the box, exposing employees to an electrical shock hazard.

29 CFR 1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.   a. Located in the basement kitchen area, two flexible cords were connected together and ran through storage shelves to supply 120 volts to the bacteria digestant pump.
