29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about February 05, 2019, at the worksite located 8200 Glen Echo Drive, Saint Louis, Missouri, Duration Construction did not initiate and maintain programs that provide for frequent and regular inspections for its subcontractor that was  installing slate roof tiles on a residential roof without fall protection.  The employee were also observed using portable ladders to access the upper landing of residential roof that had not been extended at least 3 feet above the upper landing surface. The fall distance to the ground below was approximately 18 feet.  The following hazards were noted at the site:  a/  Lack of fall protection b/  Improper ladder use
