This violation was most recently documented at a worksite located at or near 1102 N. Harrison St., ONeill, Nebraska 68763 on or about February 6, 2019. The exit door in the boiler room was blocked with an air conditioning unit that prevented unobstructed access to an exit.

This violation was most recently documented at a worksite located at or near at 1102 N. Harrison St., ONeill, Nebraska 68763 on or about February 6, 2019. The employer failed to ensure that machine guarding was used on the Hobart mixer.
