29 CFR 1910.1000(a)(2): Other substances -- 8-hour Time Weighted Averages. An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C", shall not exceed the 8-hour Time Weighted Average given for that substance any 8-hour work shift of a 40-hour work week. a) On February 6, 2019, an employee working in the Calcine Area was exposed to an 8-hour time weighted average concentration of 6.90 milligrams per cubic meter of respirable dust which exceeded the permissible exposure limit by 138% during a sample period of 320 minutes. Zero exposure was used for the unsampled portion of the shift. b) On February 6, 2019, an employee working in the Calcine Area was exposed to an 8-hour time weighted average concentration of 8.12 milligrams per cubic meter of total dust which exceeded the permissible exposure limit by 162% during a sample period of 373 minutes. Zero exposure was used for the unsampled portion of the shift. c) On February 6, 2019, an employee working in the Calcine Area was exposed to an 8-hour time weighted average concentration of 52.87 milligrams per cubic meter of total dust which exceeded the permissible exposure limit by 352% during a sample period of 446 minutes. Zero exposure was used for the unsampled portion of the shift. d) On February 6, 2019, an employee working in the Calcine Area was exposed to an 8-hour time weighted average concentration of 18.80 milligrams per cubic meter of total dust which exceeded the permissible exposure limit by 125% during a sample period of 454 minutes. Zero exposure was used for the unsampled portion of the shift. e) On February 6, 2019, an employee working in the Calcine Area was exposed to an 8-hour time weighted average concentration of 32.18 milligrams per cubic meter of total dust which exceeded the permissible exposure limit by 215% during a sample period of 375 minutes. Zero exposure was used for the unsampled portion of the shift. f) On February 6, 2019, an employee working in the Calcine Area was exposed to an 8-hour time weighted average concentration of 26.57 milligrams per cubic meter of total dust which exceeded the permissible exposure limit by 177% during a sample period of 320 minutes. Zero exposure was used for the unsampled portion of the shift.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: On or about 2/6/2019, in the Calcine Area near the stairway, a wall outlet located approximately fifty five (55) inches above the floor with a potential difference of 120 volts to ground was exposed to physical damage and did not have a protective enclosure or guard. The outlet had been damaged to the point where live wires were exposed.
