Employee(s) engaged in obtaining propane cylinders on the outside of the building to at 16910 W. 116th Street, Lenexa, Kansas to refuel powered industrial forklifts were exposed to fall hazards in that the stairway and pathway to the outside propane storage area was covered with a layer of snow and not cleared or salted.

Employees engaged in using an orange extension cord to heat up glow plugs for the diesel engine in the truck at dock 2 located at 16910 W. 116th  Street, Lenexa, Kansas were exposed to electrical hazards.  There was no ground pin on the plug end of the orange extension cord.
