29 CFR 1910.303(g)(1)(iii):     The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less.    There was not an adequate entrance to the electrical panel box as the entrance was only 23 inches wide as it was narrowed by a spare machine die.

29 CFR 1910.305(b)(1)(ii):   Unused openings in boxes, cabinets, or fittings were not effectively closed.  At a worksite located at 4000 Waco Rd, Columbia, Missouri. Employees operating Machine Press #17 were exposed to serious electrical shock and burn injuries, in that the 480 Volt to 240 Volt Acme transformer supplying power did not have a guard that effectively enclosed the electrical connections.
