29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.  EB Remodeling LLC, Tucker, GA - On or about 02/05/2019 at 4286 La Vista Road, employees were exposed to eye injuries from flying objects. Employees were not required to use eye protection when using a nail gun to install shingles on a roof of a commercial site.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  EB Remodeling LLC, Tucker, GA - On or about 02/05/2019 at 4286 La Vista Road, employees were observed exposed to fall hazards without fall protection systems, while performing roofing operations at heights of 18-feet from the lower level to the peak of the roof with a 6/12 pitch and 10.6-feet from the eave, while performing shingles installation. No fall protection systems used by employees.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  EB Remodeling LLC, Tucker, GA - On or about 02/05/2019 at 4286 La Vista Road, employees were observed exposed to fall hazards without fall protection systems, while performing roofing operations at heights of 18-feet from the lower level to the peak of the roof with a 6/12 pitch and 10.6-feet from the eave, while performing shingles installation. No training was provided to employees on the proper use of fall protection.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.  EB Remodeling LLC, Tucker, GA - On or about 02/05/2019 at 4286 La Vista Road, employees were using a shingle ladder with a nylon instead of the wire rope recommended by the manufacturer. The employer did not install the shingle ladder's wire in accordance with the manufacturer's instructions and recommendations.
