29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite, 2030 Mackenzie Way, Cranberry, PA: On or about February 7, 2019, during sign replacement activities, the employer did not ensure that appropriate fall protection systems/devices were provided where a fall hazard of at least approximately 12 feet, 5 inches existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Jobsite, 2030 Mackenzie Way, Cranberry, PA: On or about February 7, 2019, during sign replacement activities, the employer did not ensure that the ladder used to get up/down from a roof was extended at least 3 feet above the roof line.
