Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 12/19/2018, employees of Nelson Juarez were installing a new roof on residential buildings at 702-706 Hyde Drive, OFallon, Missouri.  Employees were installing shingles approximately 21 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.
