29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Press Room Delivery Conveyor: On or about February 9, 2019 and times prior, employees lifted piles of news paper waste from the horizontal belt conveyor to discard in the waste bin.  The rollers between the horizontal conveyor and the attached vertical conveyor were not guarded to protect employees from the caught in hazards.    b) Mail Room Overhead Conveyor: On or about February 12, 2019 and times prior,  employees walked underneath a low hanging overhead conveyor system to access the exit door or the vending machines.  The rollers tail rollers and the shifting metal plates of the conveyor were not guarded to protect employees from the caught in hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i:    a) Newspaper press: On or about February 12, 2019, and times prior, employees used compressed air to blow paper dust and debris out from under the press and conveyors. The air gun was not equipped with a safety nozzle and the compressor was set at 80 p.s.i.

29 CFR 1910.1200(e)(1):   The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:      (a) West Virginia Newspaper Publishing Company:  On or about February 12, 2019 and time prior, employees use printing ink, acids, glues and WD-40. The employer has not developed or implemented a written hazard communication program.        ABATEMENT NOTE:  A written program shall include a description of how the criteria for the following will be met:   1.        Labeling and other forms of warnings;   2.        Safety Data Sheets;   3.        Employee information and training.        Additionally, a list of hazardous chemicals known to be present in the workplace must be compiled.  Methods used to inform employees of the hazards associated with non-routine tasks and the informing of contractors of workplace hazards, including a description of the labeling system used in the facility and the availability of safety data sheets must also be addressed.  The written program must be made available upon request.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  West Virginia Newspaper Publishing Company, Inc.:  On or about February 12, 2019 and times prior, employees used chemicals such as inks, glues, acids, and WD-40 during printing and maintenance operations. The employer has not provided employees with information and training on the hazards of the chemicals used.
