On or about February 13, 2019, located at 2315 Reynoldsburg-New Albany Road in Blacklick, Ohio, employees were connecting a water line while in an unprotected trench that was between 7-1/2 and 11 feet deep. The spoil pile was not at least two feet from the edge of the excavation and there were no retaining means to prevent soil from falling into the trench, thereby exposing the employees to a cave-in hazard.
