29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks:     A)  East side of ISS 300 machine, on or about February 13, 2019, and or at times prior to, the conveyor belt going over a drum was not guarded, thereby exposing employees to an in-running nip point and caught-in hazard.     B)  East side of SGM machine, on or about February 13, 2019, and or times prior to, the conveyor belt going over a drum was not guarded, thereby exposing employees to an in-running nip point and caught-in hazard.

29 CFR 1910.28(b)(3)(iv): Each employee was not protected from falling into the ladderway platform hole at the entrance to the hole by a self-closing gate or offset.     A)  Zorba Platform, on or about February 13, 2019, and or at times prior to, the access area on the platform to the ladder did not have a self-closing gate, nor was there an offset in place, thereby exposing employees to a fall hazard of approximately seven feet.      B)  Zurick Platform, on or about February 13, 2019, and or at times prior to, the access area on the platform to the ladder did not have a self-closing gate, nor was there an offset in place, thereby exposing employees to a fall hazard of approximately six-feet.       Scrap Partners was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.28(b)(3)(iv), which was contained in OSHA inspection number 1306732, citation number 1 , item number 2 and was affirmed as a final order on November 5, 2018, with respect to a workplace located at 500 Addlestone Dr., Metter, Ga.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:   A)  Catwalk on north east side of the Trommel, on or about February 13, 2019, and or at times prior to, an open side edge of the walking/working platform was not guarded, thereby exposing employees to a fall hazard of approximately 15 feet.   B)  Catwalk on east side of ISS 300 machine, on or about February 13, 2019, and or at times prior to, a section of the guardrail system did not have a mid-rail, thereby exposing employees to a fall hazard of approximately 10 feet.
