On or about February 3, 2019, the employer did not ensure one or more methods of machine guarding was adequately provided to protect machine operators from hazards created by the point of operation and rotating parts of a Blanchard Grinder Machine, known as machine Number 65. c. On or about February 3, 2019, the employer did not ensure the affixed barrier guard protecting the point of operation of the Blanchard Wet Grinder Machine No. 1096 created accident hazard to the machine operators hands and fingers when the magnetic table the guard is affixed too is closed against door barrier guard.
