Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) On or about 01/16/2019 - 289 Colonial Dr. Oxford, AL, employees were exposed to a fall hazard while removing and installing shingles on a commercial building approximately 54 feet above the ground level.
