29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about February 14, 2019, on the 5:12 pitch roof of building #7, four roofing employees removing old roofing material were exposed to 27 feet to 33 feet fall hazards from the roof eaves in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about February 14, 2019, at the southwest corner of building #7, the roof access ladder did not extend 3 feet above the roof eave, exposing employees to a 27 feet fall hazard at the top of the ladder.
