29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Bragg-Mitchell Mansion (West Side: Column Area): On or about February 14, 2019, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with work performed in a Genie Z-45/25J Boom Aerial Lift thus exposing their employees to hazards such as but not limited to catapult hazards and fall hazards of up to approximately forty (40) feet to the concrete and ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Bragg-Mitchell Mansion (West Side: Column Area): On or about February 14, 2019, the employer failed to ensure exposed employee(s) were using a body belt with lanyard attached to the approved anchor point while using a Genie Z-45/25J aerial lift to perform paint activities; exposing employee(s) to catapult hazards and fall hazards of up to approximately forty (40) feet to the concrete and ground below.
