29 CFR 1910.178(m)(1): Trucks shall not be driven up to anyone standing in front of a bench or other fixed object:    Warehouse, east, stationary packing tables and movable staging racks: on or about February 19, 2019, employees (Packers) working in between the tables and racks were exposed to being struck-by moving forklifts where palletized loads were driven up to and placed on racks in the employees' immediate work areas.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that:  1. is work-related per 29 CFR 1904.4(a)(1); and,   2. is a new case per 29 CFR 1904.4(a)(2); and,   3. meets one or more of the general recording criteria of �1904.7 or the application to specific cases of ��1904.8 through 1904.12 per 29 CFR 1904(a)(3):  On or about February 19, 2019, the employer failed to maintain required OSHA 300 form "Log of Work-Related Injuries and Illnesses" to accurately record any work-related injuries that occurred in the workplace.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
