29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck  On or about 20 February 2019 employees were operating Powered Industrial Trucks without having been certified by the current employer.

29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer. (A)  Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, and/or altered by addition of extra parts not provided by the manufacturer or by the eliminations of parts.  On or about 20 February 2019 the employer placed a towing pinitol on the blade of a Powered Industrial Truck (PIT) that employees were allowed to use to move combo towers
