29 CFR 1926.850(e): It had not been determined if any type of hazardous chemicals, gases, explosives, flammable materials or similar dangerous substances had been used in any pipes, tanks, or other equipment on the property. The presence of such substances were apparent and the hazard was not eliminated before demolition was started. 241 Main Street, Sturbridge, MA: On or about February 22, 2019 the employer had not eliminated the hazard of flammable liquids contained in a diesel fuel supply pipe prior to conducting demolition activities.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (Construction Reference: 1926.59) 241 Main Street, Sturbridge, MA: On or about February 22, 2019 employee training did not include the measures employees can take to protect themselves from chemical and physical hazards posed by flammable liquids including but not limited to specific procedures and work practices to minimize contact with flammable liquids, personal protective equipment to be used, avoidance of sources of ignition and emergency procedures to be followed while dismantling flammable storage tanks and associated piping. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
