29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  (a) On or about February 21, 2019, the employer did not ensure that employees  voluntarily using half face respirators provided by the employer were medically cleared to wear a respirator. 29 CFR 1926.1101(k)(9)(vi): Training for employees performing Class IV operations did not include available information concerning the locations of thermal system insulation and surfacing asbestos containing material and/or presumed asbestos containing material, asbestos-containing flooring materials, or flooring material where the absence of asbestos has not yet been certified; and instruction in recognition of damage, deterioration, and delamination of asbestos containing building materials; and did not take at least 2 hours:    a) On or about February 21, 2019, the employer did not ensure that employees conducting debris removal and demolition of spaces containg ACM and PACM were protected from asbestos exposure where they were not trained in accordance with OSHA requirements.
