On or about February 22, 2019, at the above addressed jobsite, employees were conducting sewer operations in a trench measured at ten (10) feet in depth. The trench was not provided adequate cave in protection, thereby exposing the employees to cave in hazards. Bridges Excavating, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1244944, citation number 1, item number 1 and was affirmed as a final order on September 13, 2017, with respect to a workplace located at 1571 E. Golf Road, Schaumburg, IL 60159.
