29 CFR 1910.22(d)(1): (1) Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition:    a) Distribution facility:  On March 5, 2019 and at times prior thereto employees walking and working in the area of the northwest roll up door, were exposed to a tripping hazard.  A flooring material was not even and access plugs were not in place.        Note) Certification of corrective action is required for this item.

29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:    a) Distribution facility: On March 5, 2019 and at times prior thereto employees walking and working in the area of the  southeast roll up door, were exposed to a tripping hazard.  A floor hole at the corner of the roll up door was not marked or covered. It measured ten by five by two inches deep.        Note) Certification of corrective action is required for this item.
