Hackensack Ave., Kearny, NJ - Production area    Fendall Porta Stream personal eye wash stations were not properly maintained where employees were exposed to corrosive materials including but not limited to, Neo-Chlor 90 and Caustic soda liquid (25%). 29 CFR 1910.1200(e)(1): The employer did not implement a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a)  100 N. Hackensack Ave., Kearny NJ    The employer did not implement their hazard communication program for an employee performing a non-routine task which involved pumping a 275 gallon tote containing a mixture of water and approximately 75 pounds of Trichloro-s-triazinetrione.  On 2/22/19, the employee was injured when the adjacent tote which contained the same product exploded and the employee was struck by metal debris.      Condition was observed on or about 2/22/2019

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  100 N. Hackensack Ave., Kearny NJ  An employee pumping a 275 gallon tote containing a mixture of water and approximately 75 pounds of Trichloro-s-triazinetrione was not provided with effective training on the explosion hazards associated with this non-routine task.  On 2/22/19, the employee was injured when the adjacent tote which contained the same product exploded and the employee was struck by metal debris.    Condition was observed on or about 2/22/2019
