29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about February 25, 2019, at a worksite located at 2211 Medina Road, Medina, OH 44256, during commercial roofing work, employees were  exposed to a falls of approximately 14.8 feet.  The employees were performing roof installation activities on a commercial building low slope roof with no fall protection system/device.    JBR Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 987652, citation 01, item number 001 and was affirmed as a final order on 10/7/2014, with respect to a workplace located at 68501 Addie Way, Saint Clairsville, OH 43950.    JBR Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 314416439, citation number 02, item number 001 and was affirmed as a final order on June 16, 2010, with respect to a workplace located at 969 North Court, Medina, OH 44256.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:     On or about February 25, 2019, at a worksite located at 2211 Medina Road, Medina, OH 44256, during commercial roofing work, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.
