29 CFR 1915.116(d): Tag lines were not provided on loads that were likely to swing or to need guidance:  a) Enbridge Dock - Ogdensburg Pier Road, Superior, WI; On February 21, 2019 a tag line was not utilized in order to keep employees a safe distance from a load of steel plates being lowered onto the ground. The rigging failed and an employee was struck by the load.

29 CFR 1915.116(f): Slings were not padded by means of wood blocks or other suitable material where they passed over sharp edges or corners of loads:  (a) Enbridge Dock - Ogdensburg Pier Road, Superior, WI; On February 21, 2019 the nylon slings utilized when lifting steel plates were not adequately padded by suitable materials where they passed over sharp edges or corners of the load as the steel plates being lifted cut through the nylon slings (s/n 47921 & 56537) striking and injuring an employee near the load.
