29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Where machines were operated, including band saws, lathes, milling machines, CNC machines, a lockout/tagout program, including written lockout procedures, was not developed and implemented, including training employees performing lockout work, such as changing blades and other maintenance, locking out machines during maintenance work, and conducting annual, documented inspections (audits) of the program. Abatement documentation is required for this violation.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: Forklift training, according to the requirements of the standard, including a "classroom" portion, demonstration and explanation of forklift operation, and evaluation, was not provided. The classroom, or some formal instruction, would include hazards of forklift operation, lowering a load and/or forks when dismounting, conducting a daily (or before a day's use), documented check/inspection of the lift, and similar hazards and requirements. Evaluation would require observations of the newly trained employee while operating the forklift. All components of training must be documented and a re-evaluation conducted every three years or after an incident. Abatement documentation is required for this violation.
