The company's management employee(s) did not do the following:    1)  Conduct a pre-job meeting to review all applicable lockout / tagout procedures for the job.  2)  Obtain a copy of the contractor or temporary worker's lockout / tagout program.  3)  Obtain a copy of the contractor or temporary worker's lockout / tagout program training documentation.  4)  Determine if the contractor or temporary worker had lockout / tagout devices to isolate and control all energy sources.
