29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) West side of the building: On or about February 27, 2019, and at times prior, the employer exposed employees to fall hazards of 17 feet, in that employees were installing metal siding while working from a homemade scaffold platform which was supported by a Lull model 844C-42, which was not specifically designed for this type of work.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) West side of the building: On or about February 27, 2019, and at times prior, the employer exposed employees to fall hazards of 17 feet, in that employees were installing metal siding while working from a homemade scaffold platform which was supported by a Lull model 844C-42, and the platform was not attached to the forks by any type of securing means which would prevent it from being dislodged during operations.
