29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On or about 27 February 2019, and at times prior thereto, four rain gutter installers were performing overhead work without the use of hard hats, exposing the workers to the hazard of being struck by falling tools, materials or debris.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.  On or about 27 February 2019, and at times prior thereto, a worker was installing new rain gutters on a two-story roof without utilizing fall protection, exposing the worker to a fall hazard of approximately 16 feet to the ground below.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per attached employee.  On or about 27 February 2019, and at times prior thereto, two rain gutter installers were on a two-story roof utilizing fall protection equipment with a single anchor point rated for one-person, exposing the installers to a fall hazard of approximately 16 feet to the ground below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.  On or about 27 February 2019, and at times prior thereto, two rain gutter installers were performing work while on the two-story roof, utilizing a harness, rope and anchor. The rope was approximately 50 feet in length, while the roof edge was approximately 16 feet in height, exposing the two workers to a fall hazard from too much slack in the rope/lifeline.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about 27 February 2019, and at times prior thereto, two workers were utilizing an extension ladder to access a two-story roof without being extended at least three feet above the upper landing, exposing the workers to a fall hazard of approximately 16 feet to the ground below.
