Yard Area - On or about and times prior to February 27, 2019, a Hyundai forklift (model: 70D-7E) was operated with a non-functional audible reverse alarm, an inoperable horn, and an inoperable forks positioner system, thus exposing employees to a struck-by hazard.    b). Yard Area - On or about and times prior to February 27, 2019, a Hyster Lift Truck (model: H120FT) was operated with a non-functional audible reverse alarm and with two worn front tires, thus exposing employees to a struck-by hazard.    c). Yard Area - On or about and times prior to February 27, 2019, a Toyota Forklift Truck (model: 7FDAU50) was operated with an inoperable horn and with two worn/damaged rear tires, thus exposing employees to a struck-by hazard.    d). Yard Area - On or about and times prior to February 27, 2019, a Toyota Forklift Truck (model: 7FDKU40) was operated with a non-functional audible reverse alarm, an inoperable horn, and with four worn/damaged front tires, thus exposing employees to a struck-by hazard.
