29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On or about 05/09/2019, the employer failed to certify to OSHA that the following hazards have been abated:  (a) Citation 1, Item 1 - First floor interior: On or about 2/28/2019, and at times prior, the employer exposed employees to struck-by hazards while operating an electric Makita 7 1/4" circular saw, in that protective eyewear was not provided to employees engaged in lumber cutting operations.  (b) Citation 1, Item 2 - Roof of the structure: On or about 2/28/2019, and at times prior, the employer exposed employees to fall hazards of up to twenty-two feet, five inches, in that the use of personal fall arrest systems was not enforced while the employees were roofing the top of a two story new-build house.  (c) Citation 2, Item 1 - Company-wide: On or about 2/19/2019, and at times prior, the employer exposed employees to fall hazards, in that fall protection training was not provided prior to working at heights above six feet.

On or about 05/09/2019, the employer failed to provide documentation to OSHA demonstrating that the following hazards have been abated:  (a) Citation 1, Item 2 - Roof of the structure: On or about 2/28/2019, and at times prior, the employer exposed employees to fall hazards of up to twenty-two feet, five inches, in that the use of personal fall arrest systems was not enforced while the employees were roofing the top of a two story new-build house.
