29 CFR 1910.134(g)(1)(i)(A): The employer did not prohibit the growth of facial hair of employees using tight fitting face piece respirators which would prevent a seal between the face piece and the face:    On or about February 26, 2019, an employee was observed not being clean shaven when working on an oil & gas rig site having the potential of there being an H2S release.  The employee was exposed to a potential health hazard.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    On or about February 26, 2019, the rig manager's trailer had broken or missing covers/faceplates on several electrical outlets on the outside of the trailer.  Employee was exposed to electrical shock and/or burn hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.    On or about February 26, 2019, in the SCR house there were some panel used to cover electrical components and wiring with gaps/openings on along the edges which could allow debris and other foreign objects to enter and possibly creating an electrical short.  Employees were exposed to the hazard of electrical shock or burns.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about February 26, 2019, the CSHOs observed an electrical cable with the outer insulation pulled away from its fitting, exposing the inner conductors to damage.  Employees were exposed to electrical shock hazards.
