29 CFR 1926.453(b)(2)(iv): Employees did not always stand firmly on the floor of the basket, and not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position:  (a)   This violation occurred on or about March 1, 2019, at a job site located at 485 Market Place Blvd, Suite 140, Forney, TX; where employees were applying caulking on the outside walls of Mod Pizza restaurant while working from a JLG lift, 600AJ, PT5W95, MS18008.  At least one (1) employee was standing on the rail of the aerial lift and was exposed to fall hazards.

Employees were exposed to fall hazards.

29 CFR 1926.501(b)(10): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)   This violation occurred on or about March 1, 2019, at a job site located at 485 Market Place Blvd, Suite 140, Forney, TX; where employees were applying caulking on the outside walls of Mod Pizza restaurant while working on top of the awning.  Employees did not have on safety harnesses and lanyards and were exposed to fall hazards.
