29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) West side of  roof, on or about 2-28-19:  Two employees were working on a sloped roof (10:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 9 feet 4 inches.  ABATEMENT DOCUMENTATION REQUIRED FOR THIS ITEM

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) West side of building, north end, on or about 2-28-19:  One employee was using a portable aluminum multi-function ladder for roof access/egress, with side rails that were 24 inches below the landing surface, exposing the employee to a fall hazard of 9 feet 4 inches to the ground below.     b) West side of building, south end, on or about 2-28-19:  One employee was using a portable aluminum multi-function ladder for roof access/egress, with side rails that were 16 inches below the landing surface, exposing the employee to a fall hazard of 9 feet 4 inches to the ground below.
