On or about 5 March 2019, and at times prior thereto, a spreader bar of unknown origin with no manufacturer noted nor load test documentation, was being used by employees to lift loads weighing up to approximately 3-tons within the plant floor, exposing employees to the hazard of being struck by falling equipment and loads.
